Treatment of Necrotizing Fasciitis with Sore in Spinal Cord Injury Patient: A Case Report

ABSTRACT
Chronic pressure ulcer is very common clinical problem especially in immobilized patients. It usually needs short- and long-term treatment. Approximately 85% of patients with a spinal cord injury (SCI) develop pressure ulcers during their lifetime. Despite several surgical approaches such as direct closure, skin grafting, and skin or musculocutaneous flaps, the rate of postoperative complications, especially ulcer recurrence, remains high. Necrotizing soft tissue infections from decubitus ulcers in patients with SCIs are of particular concern, given their very rapid progressing & potentially life-threatening nature. This life-threatening complication is characterized by extensive, rapidly progressive necrosis of the fascia, followed by necrosis of the subcutaneous tissue, and muscle with an overall mortality rate of 25% to 30%. A case report of the successful wound healing with aggressive surgical debridement, VAC dressing and local flap in paraplegic patient who suffered from nedrotizing fasciitis from grade IV sore is described.
