Physical Examination and Management of Lower Extremity Ulcers

ABSTRACT

Success in managing lower extremity ulcers requires a total patient commitment. An underlying disease of vascular pathophysiology and its contribution to lower extremity ulcers is critical in the management of the ulcers. Arterial reconstruction is the hallmark of treatment for arterial disease. In general, dry, fixed, stable, dry eschar should be kept dry until the tissue is revascularized. Venous ulcers always require external compression, ultimately in the form of compression stockings. Diabetic foot ulcer management should be focused on off-loading of pressure, infection control, safe and complete sharp debridement, clean and moist condition. All of them are a challenge for both the patient and the health care provider. A team approach with total involvement of the health care system and the necessary compliance of the patient will be the infrastructure for achieving better outcomes of care.
