Two-year Disease-free Protocol with Surgical Treatment on Stage IV Pressure Ulcer

ABSTRACT
To introduce the two-year disease-free surgical protocol on pressure ulcer. We designed the protocol on preoperative, intraoperative and postoperative care based on some literatures and medical records of our center. Then, we put 11 patients with good compliance into this protocol from 2009 to 2010. Two of them abandoned to follow the protocol. The protocol covered surgical methods, bacteriologic control with antibiotics, and postoperative limitation of positional change, weight bearing, and emotional support. The surgical methods had two-step. The bacteriologic study with smear culture and tissue culture was performed with swab from preoperative surface of wound, tissue got during the debridement, swab from second preoperative surface of wound base, and drainage from the Hemovac. They could be allowed to change of his or her body position by oneself from 21 days after operation. They were allowed to get the weight bearing position from 28th day after the operation. There were no events of recurrence during admission period, and outpatient clinic visits at each time of first, third, sixth month, and one-year after the operations. All nine patients had no recurrence from now on. This study showed selective indications for surgery and proper protocol could produce no recurrence after surgical treatment.
