Efficacy of Curavac System to Enhance Skin Graft Survival in Chronic Intractable Wound

ABSTRACT
In patients with poor general condition, the management of chronic intractable wounds is considered to be difficult and still remains a challenge. With the introduction of the Curavac(Daewoong Pharmaceutical, Korea) in 2009 that consists of the wall suction unit or the private label product of portable suction unit, expensive application of V.A.C. Therapywas solved by reducing additional products as well as foam. Because the treatment with this device was registered for health insurance benefits, it was developed as an alternative option to reduce the burden of medical expenses. In this study, we have cured 20 patients with chronic wounds after successful skin graft by the use of Curavacpostoperatively. The average survival rate was 98.6 percent and adjacent infection on graft margin was not observed except one patient. One of the advantages of the Curavac is to enhance a good contact between skin graft and recipient bed without any dead space. Second, there is no need of splint or additional devices to be fixed. Third, this treatment option is very cost-effective because the health insurance benefits. Fourth, there are no differences compared to V.A.C. Therapy. Fifth, it continuously absorbs either hematoma and seroma. Sixth, it increases the rate of successful skin graft by both accelerating neovascularization and preventing infection. Furthermore, it improves the healing time in case of meshed skin graft as well. Lastly, this can be a good alternative dressing method technically, especially for difficult surgical areas. Therefore, the application of Curavac after a skin graft is assumed to be a very efficient treatment modality in chronic intractable wounds. (J Korean Wound Management Soc 2010;6:18-24)
