ABSTRACT |
Extensive soft-tissue defects of lower leg caused by traumatic injury often require coverage by skin grafts. It allows rapid and reliable coverage of defects without significant functional donor-site morbidity. However, the take of the of the skin grafts is limited by the infection. Scarring and contracture are major long-term sequele of meshed split-thickness autograft for full-thickness skin injury. We performed a graft with a acellular human dermal graft material (AlloDermⰒ; LifeCell Corporation, The Woodlands, TX) overlying meshed split-thickness skin autograft in 5 patients for the treatment extensive traumatic lower leg skin defects between January 2004 and October 2005. Before the skin grafts, wound bed preparation had been carried with silver-containing hydrofiber dressing (Aquacel AgⰒ; ConvatecⰒ, London, UK) including surgical debridement. All donor sites were dressed with 2 layers of Aquacel AgⰒ and floppy gauze on the top of the primary dressing. The mean age of the patients was 32.2 years. The overall Aquacel AgⰒ treatment period was 10.3 days. In all patients, AlloDermⰒ exhibited a complete take . There was no incidence of graft failure and partial loss related with infection. Donor site infection and other complications were not sure. Inevitable serious hypertrophic scar could be averted. This study presents that AllodermⰒ successfully can be used in reconstructions for large, complicated lower leg skin defects. Aquacel AgⰒ dressing provides and promotes appropriate environment for surgical wound preparation and is shown to help reduce pain associated with wound dressing. Also it seems to be related to a low incidence of scar formation on donor site. (J Korean Wound Care Soc 2007;3:29-32) |
